What is Entry Point Durham? |
Hello and welcome back! This month we are going to shift focus a bit and introduce & explain a partner of ours, Entry Point Durham. As you might be aware, FMF is part of a larger system of homeless service providers in Durham. This system is known as a Continuum of Care (CoC) and is part of HUD’s program to “promote community-wise commitment to the goal of ending homelessness.” The CoC is designed to coordinate funding, best practices, and access to services within a specified geographic area. To ensure this access, an evidence-based coordinated intake program is mandated by HUD. Entry Point is the "front door" to Durham’s coordinated intake, ensuring equitable and easy-to-navigate access to the homeless services resources of the Durham community. Entry Point is staffed by a trained team who are available to listen to and assess households facing or experiencing homelessness. Staff evaluate a household's strengths, needs, preferences, and barriers to housing.
These findings may result in diversion efforts to help households retain existing housing or identify safe, temporary alternatives to being unsheltered; this is accompanied by referral to resources to address permanent housing needs and other areas, such as healthcare, transportation, and public benefits. If a household cannot be diverted, they are placed on the waiting list for shelter. At FMF, we welcome Entry Point as a valuable partner. They are the first point of contact for members in the community and ensure that people in need of services are fairly and equitably prioritized. This enables FMF to create the greatest impact possible—for our guest families and the Durham community.

Partner Spotlight: Duke School of Nursing |
This year, we've partnered with Duke University School of Nursing’s (DUSON) Mobile Prevention and Care Team (M-PACT).
For more than 10 years, students from DUSON have provided health screening and education to Families Moving Forward guests as part of their clinical experience through their Healthy Homes programming series, which helps students learn about community and public health nursing. On January 16, 2024, the partnership with DUSON expanded to include an onsite clinic for FMF guests. The M-PACT Clinic seeks to provide health promotion and health maintenance for FMF guests and referral connection to primary care.
Tammy Jacobs, M-PACT program manager, said: “We're the bridge to primary care. Our goal is to serve families who are not engaged with their own personal primary care physician at the time. We address immediate needs and connect them with resources to find primary care or specialist care.”
DUSON Clinical Professor and M-PACT Clinic Director Dr. Donna Biederman said, “Families Moving Forward has been a vital community partner over the years to help educate our students about the many determinants of health. This is an exciting time for our partnership with FMF as we launch the M-PACT Clinic in the community as a nurse-led model of care.” M-PACT is a program of the DUSON Community Health Improvement Partnership Program (D-CHIPP) and is funded by a Health Resources and Services Administration (HRSA) cooperative agreement (UK1HP46054-02). |

Meet Jasmine Rivera, FMF's new Director of Programs and Residential Services. She shares, "I'm originally from Philadelphia, but I lived in the Pittsburgh area for years before relocating to Durham. I've been here six months, and I love it. I'm excited to have this amazing opportunity at FMF, and I look forward to working with our families and community to end family homelessness in Durham." |
